4/25/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, JaCarei’s actions suddenly altered. JaCarei was a little more verbal when talking about things he likes. JaCarei seem to switch moods a lot depending on what is being asked. Behavior: JaCarei chose to distance himself from QMHP-C when reflecting on his bad experiences. JaCarei uses a lot of non-verbal cues to express how he feels. Intervention: QMHP-C conducted session on following all directions and limiting outburst of anger at home, in session, and in the community. QMHP-C focused on this topic because of the opening statements that JaCarei made pertaining to his first day of in-home services. QMHP-C utilized role-play to help JaCarei understand the importance of following all directions and limiting outburst of anger in a therapeutic or non-therapeutic setting. QMHP-C utilized active listening skills while JaCarei excitedly discussed his feelings of being in services and talking about his problems with someone he is still building a relationship with. QMHP-C educated JaCarei about using good manners at home and offering alternative methods and suggestions to model appropriate language that JaCarei should use to ask for things or to get someone’s attention. Intervention: QMHP-C processed JaCarei feelings and offered choices of what to do when faced with conflict or challenged in the home. QMHP-C will assist with reducing anger-provoking situations through self talk and role play scenarios in order to prevent escalation and dealing with anger assertively. QMHP-C will continue to assist JaCarei with managing anger and frustration by reviewing and role-playing effective coping skills. QMHP-C helped JaCarei problem solve when he began displaying negativity towards himself and others. QMHP-C modeled proper positive behavioral expectations for JaCarei to encourage additional layers of support and understanding of rules and expectations. QMHP-C encouraged positive behaviors at all times and reminded JaCarei of his goals and objectives that he is expected to achieve. Response: JaCarei was not receptive to counselor’s prompts and redirections and after a period of time resulted to behaving very erratically and becoming physically violent as well. JaCarei has a history of displaying aggressive, non-compliant behaviors towards authoritative figures in the home and/or community setting. This has been the first occasion in which JaCarei has displayed this level of behaviors in the presence of counselor. JaCarei was throwing objects, yelling loudly, at mother and counselor. JaCarei was highly oppositional during session and displayed argumentative and frustrating behavior. JaCarei still struggles with coping with stressors and requires TDT services but have shown some improvement in this area. JaCarei was acceptive of extra help and support. JaCarei utilized the resources provided and seem to be doing better with managing symptoms of ADHD. Response: JaCarei returned to activity ready to work and was more motivated to try and stay positive for the remainder of the day. JaCarei displays moderate participation unless he’s distracted. JaCarei recognized when he’s not paying attention and quickly corrected himself. Mom enjoyed seeing JaCarei do the right thing for once. After several strategies and restraint by counselor, JaCarei was able to stabilize his negative behaviors by utilizing the techniques learned during today’s session. Mom and JaCarei promised to practice techniques over the weekend and review and complete the homework assignment.
